If you’re a new client, we do require your contact details and background.
To save time on arrival, you are welcome to pre-complete the following form and bring it with you to your first appointment.

Account Payments
We ask that all accounts to be paid at the time of consultation either by cash, cheque or Eftpos. Bulk billing is available to Department of Veterans Affairs cardholders. We have available a Hicaps/Eftpos machine and we accept all Health Funds. No acceptance of American Express or Diners Cards.
Confirmation of Appointments
A SMS message will be sent to your nominated phone number the day before your appointment. If you do not have a mobile number then a phone call will be made to your landline confirming your appointment.
Missed appointments
As your time is important to us, missed appointments is also a missed opportunity for a vacancy to be filled. If you are unable to attend your appointment, please advise the reception staff as soon as possible. If contacting out of office hours, please leave a message on our phone answering service.
Specialist Treatment
Extra Corporeal Shockwave Therapy and Swift Microwave Wart Therapy is only available at the Gawler clinic.
Nail Surgery
Nail surgery is available only at our Gawler Clinic. A surgery procedure requires a follow up redress appointment the next day and subsequent checks until healed (follow-up appointments can be made at either clinic locations).
Building Access
The Barossa Valley clinic has a very large open carpark access at the rear of the building. If you require assistance in entering the building please press the buzzer located by the door handle and the Receptionist will assist promptly. Entry is also via the main front door.
The Gawler Clinic has one main entrance and a few available car spaces at the front of the building. All other parking is available in the surrounding side streets.
